Home Health Department 

 

 

Home Health Care Services

 

 

The Effingham County Health Department’s Home Health Care division provides Skilled Nursing, Certified Nursing Assistant services, Physical, Occupational and Speech Therapy, along with Medical Social Services. These services are provided in the comfort  of  the patients home during the time when the patient is primarily confined to their home under a physicians care.

This cost effective service allows the patient to receive his/her individualized care at home while recovering in

familiar surroundings.

 

  

 

Visiting Nurses

 

Visiting nurses provide skilled nursing care under a plan developed by the patient, doctor and visiting nurse. Plans may include areas such as: general assessment, teachings related to medications, safety, clarifications, diet, general care, dressing changes, wound care, catheters, ostomy care and I.V. Therapy.

 

 

Certified Home Health Aides

 

CNA’s provide assistance with personal care and bathing of the home health patients.

 

 

Physical, Occupational and Speech Therapists

 

Therapists provide rehabilitation for patients following an individualized plan of care. The therapists assist in the transitioning of the patients return to daily living activities.

 

 

 

 

 

 

FAQs

 

Q: Who is eligible for Home Health Services?

A: Anyone who is homebound due to a medical condition. Referrals are accepted from a physician, hospital, long-term care facility, family, friends and the patients themselves.

 

Q: Who pays for Home Health Care?

A: Upon eligibility, the health department will bill Medicare, Medicaid, private insurance, Workman’s Compensation insurance and Veteran’s Affairs. The health department accepts assignment of what the above payer’s reimburse; there should not be a balance for the patient to pay.

 

Q: What if I am unable to pay for Home Health Services?

A: A private pay program, based on your income, is available.

 

 

Q: Do you have to live in Effingham County?

A: The health department is licensed to provide Home Health services in the counties of 

     Effingham, Jasper, Cumberland and Shelby.
